NEW ON-LINE PAPERWORK & PHYSICAL INFORMATION
 
 
Parents and Athletes
The Athletic paperwork for the 2018-2019 school year must be completed on-line for participation in the Athletic Period as well as participation in any sport. All athletes must be approved for enrollment in the Taylor Middle School Athletic Program. Each athlete must have the on-line paperwork completed, as well as have a completed yearly athletic approved physical. Without these actions no student will be allowed to remain in the Athletic Program nor will they be allowed to compete on any Taylor Middle School athletic team. The on-line paperwork and athletic physical is due by the first day of school August 20, 2018 for all athletes regardless of the sport in which they will participate.
 
On-line paperwork may be found by going to the Taylor High School Web Page. Click on "Activities" then click on "Athletics" then click on "Athletic Forms". Here you will see the prompt for "Register my Athlete" go to the site and set up an account for each athlete planning to be in the Athletic Program. Once you have completed all of the on-line information you will be given a prompt to print the "Student Emergency Contact" the "Medical History" and the "Approved Athletic Physical" pages, all of these must be filled out by hand and turned in to Coach Dews at the TMS Athletic Office. No athlete will be allowed to participate in the Athletic Program without the on-line paperwork and the Athletic Physical, there are no exceptions to this rule.
 
Please go into the website information and get the required paperwork completed prior to the first day of classes. August 20, 2018.

Driving Directions for TMS Athletic Events:

 

Benold Middle School – Gym & Football Field (Georgetown)

*Take Hwy 95 North to Circleville. Turn left on Hwy 29 West to Georgetown. Continue 15 miles on Hwy 29 West. Turn Right at NE Inner Loop in Georgetown. Continue 4.9 miles. Take Lakeway drive to Northwest Blvd. Benold Middle School is on the left on Northwest Blvd.

 

 

Elgin Middle School (Football Field)

*Take Hwy 95 South out of Taylor. Continue 16 miles to Elgin Texas. Turn left at the first light at North Avenue C. Continue 1.0 miles to West 11th Street. Turn right on West 11th Street. The Football Field is on the left hand side of the road.

 

 

Elgin Middle School Gym

*Take Hwy 95 South out of Taylor. Continue 16 miles to Elgin Texas. Turn right at the first light at North Avenue C.   Continue .3 miles West and Elgin Middle School is on the Right hand side of the road.

 

 

Farley Middle School –Hutto (Football Field & Gym)

*Drive West on Hwy 79 to Hutto (9 miles). Turn left at the first light when entering Hutto on FM 1660. Continue 5 miles traveling South on FM 1660 for 5 miles. The destination Hutto Farley Middle School is on the right hand side of the road. The gym is to the front of the school and the football field is behind the school.

 

 

Forbes Middle School – Gym & Football Field (Georgetown)

*Take Hwy 95 North to Circleville. Turn left on Hwy 29 West to Georgetown. Continue 15 miles on Hwy 29 West. Turn Right at NE Inner Loop in Georgetown. Drive 1.9 miles and Forbes Middle School is on the left.

 

 

Hutto Middle School (Football Field & Gym)

*Drive West on Hwy 79 to Hutto. (11 miles)   Turn right at the light on Exchange Blvd. Continue driving North on Exchange Blvd for 1.5 miles. The school is on the left after the stop sign. The gym is to the front of the school and the football field is behind the school.

 

 

 

Liberty Hill Middle School Gym

*Take Hwy 95 North to Circleville. Turn left on Hwy 29 West to Georgetown. Continue on Hwy 29 West through Georgetown. Continue 15 miles West on Hwy 29. The destination Liberty Hill Middle school (13125 TX-29) is on the left. (Turn left at Traffic light).

 

 

Liberty Hill Stadium (Old Panther Stadium-Football Field)

*Take Hwy 95 North to Circleville. Turn left on Hwy 29 West to Georgetown. Continue on Hwy 29 West through Georgetown. Continue 18 miles. After crossing over railroad tracks on Hwy 29, turn left at the light onto Loop 332. Continue 1 mile and the stadium is on the left hand side of the road.

 

 

Tippit Middle School – Gym & Football Field (Georgetown)

*Take Hwy 95 North to Circleville. Turn left on Hwy 29 West to Georgetown. Continue on Hwy 29 West to South Austin Avenue (18 miles). Turn left at South Austin Avenue and continue for 2.1 miles. Turn right on Leander Road. Continue for 1.3 miles and Tippit Middle School is on the left hand side of the road.

 

 

TRAVEL REQUIREMENTS FOR ALL TMS ATHLETES
All athletes are required to travel to and from athletic activities on the bus with the team. Failure of an athlete to conform to the travel policy covered in the Athletic Handbook could result in; restricted from travel, removal from the team and possibly removal from the Athletic Program. In the case of an immediate family emergency the discretion of the coach comes into play. The coach will then contact the Athletic Coordinator and permission will follow. If there is a hardship situation there is a form that must be submitted no less than 48 hours prior to the game involved. All forms can be obtained through the coach of the team or by contacting the Middle School Athletic Coordinators Office at (512) 352 2815 ext 5212 ***Please keep in mind that all travel will be with the team and only excused if the paperwork is filled out. The paperwork should only be used in emergency or hardship situations only. We would prefer to have all Athletes traveling with the team to and from all contests.

Travel Release Form
As a parent if you need to take your child from an Athletic contest, the proper paperwork must be filled out and on file prior to the date of the contest. In case of family emergency the coach in charge at the contest will use discretion and may allow the child to be released to a legal guardian only. All athletes are required to travel with the team to and from all athletic contests unless there are extenuating circumstances and the proper steps have been taken. This is a Taylor ISD Athletic Policy and is on file with the Athletic Department as a requirement for participation in athletics. If there is an unavoidable circumstance and the student-athlete needs to ride home with a parent please utilize the form provided below for approval from the Athletic Administration. Please use the form below titled "TISD Post Activity Students Release"
