TAYLOR MIDDLE SCHOOL

ATHLETIC DEPARTMENT

Parents,

Taylor Middle School will be offering Athletic Physicals at the school on Wednesday May 9th during the 6th Grade PE classes and the 7th & 8th Grade Athletics classes. The charge for the physical is $20.00,and will be performed right here on the Middle School Campus. Students must pay at the time of the physical.

All paperwork must be filed on-line prior to the physical being given. You may access the paperwork website by going to the Taylor High School Webpage and clicking on the link on the Athletics page. Once the information is completed you can then print off the Medical History Form and the Physical Form that will be used for the process. Athletes should bring these two forms with them on May the 9th the day of the physical examination.

If you choose to not have your child’s physical done here at the school then the physical must be received prior to August 1st and must be on file with the Athletic Department.

  • 8th grade going to 9th grade the physical and paperwork must be turned in at the High School no later than August 1st.
  • 7th to 8th grade and 6th to 7th grade students physical and paperwork must be turned in at the Middle School no later than August 20th

Failure to have the paperwork processed and turned in on time will eliminate the students ability to be in the Athletic Period thus excluding them from any school sponsored Athletic Activity.

Please make sure that you have completely followed the requirements to obtain a legal physical and process the paperwork to insure that your child will be included in the TMS Athletic Program.

Thanks,
